Medical & Special Request
Our wonderful school nurse Mrs Macfarlane is always available to discuss any medical, dietary or other needs your child might have. To ensure we have the most accurate information to care for your child, can we please ask you to complete the below form if you would like use to administer any medication during the school day.
Before completing the form please ensure you have read the below information:
- I have administered any medication to my child today, I will ensure I email the date and time that this was given to Mrs Macfarlane and my child's form tutor.
- I understand that school staff are not medically trained and are not obliged to administer any medication. When medication is no longer required, I understand that it will be returned to me so that I can arrange for its safe disposal.
- I will ensure that my child does not bring any medication into school. It is my responsibility to hand any medication directly to a member of staff or to the school office.
- I agree to collect any remaining medication at the end of the school day or at the end of term, as appropriate. I understand that if I do not do so, the school will dispose of it safely.
- I accept that it is my responsibility to notify the school of any changes to the details provided on this form.
- I accept full responsibility, on my own behalf and on behalf of my child, for the administration of medication by school staff who are not medically qualified.
- I understand that the school can only accept medication that is in date, clearly labelled, and provided in its original container. Fore prescribed medication, I will ensure it has a pharmacy dispensing label.
